Consent to Share Information
Consent is needed to share Personal Health Information (PHI) - that is anything communicated in a psychotherapy session; and it is confidential. Confidential information cannot be shared or disclosed without your written consent unless it is mandated or permitted by law and regulation. Consent is not required for a provider’s insurance claim. However, I use the Consent form when you want to document therapy for a claim that you submit. The documentation goes from me to you. I use the Consent form when someone joins therapy who was not present at the outset. I use the Consent form if you want to bring a guest to session. This seldom happens, and I do not recommend it. I cannot guarantee that a guest will maintain strict confidentiality
